For use with Internet Explorer
 
1. Click either the desired Segment One or Segment Two Link (In Red
2. The contract will now appear in a new window. ( For use with Adobe reader see below)
3. Fill out the contract by typing the desired information into the gray boxes and selecting the appropriate checkboxes when required.
4. Review all of the information that was typed into the contract.
5. Go to File, then select print, and print two copies of the contract.
6. Sign your name in the appropriate box and then mail in one copy of the contract, along with a photocopy of your birth certificate (Segment One Only) and class payment.

 

 

You will receive your Level 1 License after successfully completing a Segment 1 class.

You will receive your Segment 2 certificate of completion after successfully completing a Segment 2 class.

 For use with Mozilla Firefox
 
1. If Adobe Reader is already installed skip to step two, if not visit http://www.adobe.com/products/acrobat/readstep2.html and install the newest version of Adobe reader.

2. Click either the desired Segment One or Segment Two Link (In Red).
3. A dialog box will appear, such as the one shown. (right) 
   Click on Open With Adobe Reader. If the contract
   opens in full screen, simply hit Esc to return to normal
   view.
4. Fill out the contract by typing the desired information into
   the
gray boxes and selecting the  appropriate checkboxes
   when required.

5. Review all of the information that was typed into the 
   contract.

6. Go to File, then select print, and print two copies of the
    contract.

7. Sign your name in the appropriate box and then mail in
    one copy of the contract, along with a photocopy of your
    birth certificate
(Segment One Only) and class payment.

For use with Adobe Reader

1. If Adobe Reader is already installed skip to step two, if not visit http://www.adobe.com/products/acrobat/readstep2.html and install the newest version of Adobe reader.
2. From the window with the contract visible, go to FILE, then click on SAVE AS, then select SAVE A BLANK COPY. Next give the file a name and select where on your computer you wish to save the contract to.
3. Once saved, go to where ever you saved the contract and open it. A dialog box might appear asking about a full screen, click on NO.
4. Fill out the contract by typing the desired information into the gray boxes and selecting the appropriate checkboxes when required.
5. Review all of the information that was typed into the contract.
6. Go to File, then select print, and print two copies of the contract.
7. Sign your name in the appropriate box and then mail in one copy of the contract, along with a photocopy of your birth certificate (Segment One Only) and class payment.
